Why QHSE Indicators Are Essential

“You can only improve what you measure.” This maxim is especially true in QHSE. Without reliable indicators, you’re flying blind: there’s no way to know if your actions are delivering results, to detect deviations before they become full-blown problems, or to convince your management to invest in prevention.

But beware: too many indicators kills the indicator. Here are the 15 truly essential KPIs, organized by domain.

Quality Indicators (5 KPIs)

1. Nonconformity Rate

Formula: Number of NCs / Volume of activity (units produced, orders, services delivered)

Why: This is the fundamental indicator of process control. Its trend reveals whether your quality system is improving or deteriorating.

Best practices:

  • Distinguish between internal NCs (detected before delivery) and external NCs (detected by the customer)
  • Track the trend over a rolling 12-month period, not just the monthly figure
  • Analyze by process, not just at the aggregate level

2. Customer Complaint Rate

Formula: Number of complaints / Number of active customers (or revenue)

Why: This is the voice of the customer. A customer who complains is giving you a chance to correct the issue. The one who says nothing and leaves is lost forever.

Best practices:

  • Categorize complaints (product, service, delivery, communication)
  • Measure the handling time for each complaint
  • Track the satisfactory resolution rate

3. Average Corrective Action Closure Time

Formula: Sum of closure times / Number of closed actions

Why: Identifying a problem is good; resolving it is better. This KPI measures your responsiveness and your ability to follow through on actions.

Target: Variable depending on the nature of the actions, but on average < 30 days for standard corrective actions.

4. Internal Audit Compliance Rate

Formula: Number of compliant criteria / Total number of audited criteria x 100

Why: It reflects the maturity of your management system. An upward trend shows your system is improving.

5. Customer Satisfaction

Formula: Average score from satisfaction surveys (or NPS, or customer retention rate)

Why: This is the ultimate purpose of your entire quality system. All other indicators contribute to this one.

Safety Indicators (5 KPIs)

6. Lost Time Injury Frequency Rate (LTIFR)

Formula: (Number of lost-time injuries x 1,000,000) / Number of hours worked

Why: This is the benchmark indicator, recognized internationally. It enables comparisons between companies and across industries.

Benchmarks:

  • Heavy industry: LTIFR < 20 is a good result
  • Service sector: LTIFR < 5 is expected
  • Excellence: LTIFR = 0 over 12 months

7. Severity Rate

Formula: (Number of lost days x 1,000) / Number of hours worked

Why: LTIFR alone is not enough. A stable LTIFR with a rising severity rate means accidents are less frequent but more serious – which is concerning.

8. Number of Near Misses Reported

Why: This is a proactive indicator. For every accident, there are on average 30 near misses and 300 unsafe conditions (Bird’s pyramid). If your teams report few near misses, it’s a sign of under-detection, not perfect safety.

Target: The near-miss to accident ratio should be > 10. If it’s < 5, your reporting system isn’t working.

9. Prevention Action Completion Rate

Formula: Number of actions completed on time / Number of planned actions x 100

Why: Planning prevention actions is easy. Completing them on time is what makes the difference.

Target: > 80% completed on time.

10. OHS Training Coverage Rate

Formula: Number of trained employees / Number of employees requiring training x 100

Why: Competence is the primary lever for prevention. Electrical certifications, equipment operation licenses, first aid, ergonomics… training tracking is crucial.

Environmental Indicators (3 KPIs)

11. Specific Energy Consumption

Formula: Energy consumption (kWh) / Unit of production (ton, m2, piece)

Why: Gross consumption alone is insufficient (it varies with production volume). The specific indicator measures your actual energy efficiency.

12. Waste Recovery Rate

Formula: Weight of recovered waste (recycled, reused, energy-recovered) / Total weight of waste x 100

Why: It measures your contribution to the circular economy and can have a significant financial impact (disposal costs vs. recycling revenue).

13. Number of Environmental Incidents

Why: Spills, leaks, emission threshold exceedances, neighborhood complaints… every environmental incident is a warning signal that requires action.

System Indicators (2 KPIs)

14. Audit Program Completion Rate

Formula: Number of audits completed / Number of audits planned x 100

Why: An audit program that’s not followed through is a sign of disengagement or lack of resources. It’s also a point that certification auditors systematically check.

15. Percentage of Management Review Actions Completed

Formula: Actions completed / Actions decided during management review x 100

Why: The management review is the strategic cockpit of the QHSE system. If the decisions made there are not followed up with action, the entire system loses credibility.

How to Build an Effective QHSE Dashboard

Rule 1: Less Is More

15 indicators is the maximum for an operational dashboard. Beyond that, you won’t track them. Management doesn’t want 50 KPIs – they want the 5-7 that truly matter for the strategy.

A single data point means nothing. It’s the trend over 6-12 months that is meaningful. Use trend charts, sparklines, and variation arrows.

Rule 3: Define Alert Thresholds

For each indicator, define:

  • Target (the goal to achieve)
  • Warning threshold (attention needed, action to plan)
  • Critical threshold (immediate action required)

Use a color code: green / yellow / red.

Rule 4: Automate Data Collection

If your indicators require manual collection, they will always lag behind. Automate data gathering from source processes as much as possible.

Rule 5: Communicate at Every Level

  • Executive leadership: Strategic dashboard (5-7 KPIs, macro view, trends)
  • Managers: Operational dashboard (10-15 KPIs, detail by department)
  • Shop floor: Visual indicators (workshop displays, screens in common areas)

Conclusion

QHSE indicators are not an end in themselves. They are a means to steer, decide, and communicate. The 15 KPIs presented here form a solid foundation for any organization – to be adapted, of course, to your context, industry, and priorities.

The key: choose few indicators but track them rigorously. An indicator that is neither analyzed nor communicated is a useless indicator.
